"Understanding Hospice and Helping Patients
Gain Access to Services"
An extensive look at what hospice care is in 2022 and the services offered by the Medicare Hospice benefit. Education is aimed to help healthcare providers gain a comprehensive understanding of hospice. Referral to hospice does not have to be an abrupt transition from disease-targeted care to comfort care. Three care models -- Palliative Care, Open Access, and Hospice -- can provide patients faced with chronic, and ultimately life-threatening illness, a smooth transition as they proceed through the continuum of care. Clarification is provided on what differentiates hospice and palliative care.
Objectives:
• Describe the Medicare Hospice benefit and the prognostic criteria required for admission to hospice.
• Explain the differences between hospice and palliative care.
• Differentiate between the levels of care provided by hospice.
• Describe the admission and certification process for hospice.
• State services covered by hospice.
Education Credits will be offered: Seasons Hospice & Palliative Care is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Centers Commission on Accreditation. Seasons Healthcare Management Inc., Seasons Hospice & Palliative Care, is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program Organizations. State and provincial regulatory boards have the final authority to determine whether an individual course m ay be accepted for continuing education credit. Seasons Hospice & Palliative Care maintains responsibility for this course. ACE provider approval period: 1/20/2022 to 1/20/2025. Social workers completing this course receive 1 clinical hour of continuing education credits.
